Early Warning Services is a consumer agency for reporting. All the information of your banking activity, including transaction history, is kept in check. Writing to request the Early Warning Services company to investigate and remove inaccurate information that is in your consumer report is the only way to clear any false and derogatory information.

You can fill the form by adding your particulars on the left side of the document in the following order:
● Full Name (eg. John Doe)
● Address Line 1 (eg. 123 Example Street)
● Address Line 2 (eg. New York, NY 12345)
Next, you will have to enter your document date (eg. 01/25/2021).
Then you need to scroll down and enter your personal information as they appear on your Driver’s License or ID. Enter the details in the following manner:
● Full Name (eg. John Doe)
● Social Security Number (eg. 123–45–6789)
● Date of Birth (eg. 10/30/1975)
● Address (eg. 123 Example Str, Unit 1, New York, NY 12345)
● Phone Number (eg. 123–456–7890)
● Email (eg. example@example.com)
Then in the text of ‘Sincerely’, you will be required to type in your First and Last name again (eg. John Doe).
After that, you will be required to upload copies of your identification:
● Upload the copy of your Driver’s License on the left side of the document;
● Upload the copy of your Social Security Card on the right side of the document.
Now, you can scroll to Page 3 of the document and enter details in the table under the heading that says ‘Exhibit A’.
Enter the disputed items on the left and the account number or identifier on the right side. You can follow the following order to get a better insight:
- An account with the dispute (eg. Creditor Example LLC) on the left and the account number on the right side (eg. Account # 1234567890);
- Debt Collector Example LLC on the left of the table and then add the reference number on the right (eg. Reference # 1234567890);
- Bank Example LLC, Unauthorized Inquiry on the left of the table and the date of inquiry on the right (eg. January 1, 2021);
- You can also dispute your address, by entering the address on the left side (eg. 111 Example Street, Apt 1, Miami, FL 67890) and the reason for its dispute (eg. Erroneous Address).
Then you can scroll down to the ‘Notary Section’ and write your first and last name again (eg. John Doe). Make sure to get your document notarized after clicking on the Buy button and getting your document printed.
Finally, mail your document by USPS or Letter Carrier online mailing service.
