Equifax Dispute — Method Of Verification Report

If you have sent any kind of dispute letter to Equifax before, then it is quite understandable that you might face confusion, or be totally blank about what to expect next. It is quite easy, but the thought of going through extensively complicated processes might make you rethink your decision.

Start filling up your form by entering your personal information first.
● At the top, write down your First and Last name. Example: John Doe;
● Then enter your Current Address — keeping in mind that this address must be the same as the one mentioned in your ID or Driver’s License:
Address Line 1: 123 Example Street, Unit 1
Address Line 2: New York, NY 12345
● Then choose your Document Date. This implies the date on which you are filling the form;
● Now scroll down and once again enter your First and Last Name in the same way as you did above;
● Then, input your Social Security Number (e.g. 123–45–6789);
● Enter your correct Date of Birth: 10/30/1975;
● Then once again, enter your Current Address, this time in a single line (e.g: 123 Example Street, Unit 1, New York, NY 12345);
● Now, enter your Phone Number (e.g: 123-456-7890) and your Email address: email@example.com;
● On the same page, where the letter is ending, under ‘Sincerely’, you need to enter your First and Last Name again.
Proofs of Identification
In the next section, you are required to submit two different forms of identification documents. You can upload your ID or Driver’s License on the left-side ‘Upload’ section. On the right side, upload your Social Security Card.
Please scroll down. You will come across the Notary Section. Here, you will enter your First and Last Name again.
Details of Disputed Items
Now scroll down to the fourth page, ‘Exhibit C’. You can start adding all the details that you have regarding your disputed items. Here is how you can fill in the table provided:
● Add ‘Creditor Example LLC’. Then, Account # 123456;
● Next, ‘Debt Collector Example LLC’. Reference # 567890;
● Then, ‘Bank Example LLC’, Unauthorized Inquiry. Date of Inquiry: 11/01/2019.
In this way, you can keep on adding details of all the items against which you want to raise a dispute.
Types of Disputed Items
Always remember that it is your legal right to dispute anything that is being reported illegally on your Equifax report. The Credit Bureau is bound to comply with your requests under the Fair Credit Reporting Act (FCRA).
Any wrong address, employer name, or any invalid information reported on your file could be disputed (e.g. 111 Example Street, Apartment 2, Miami, FL 67890, Erroneous Address).
Printing the Form
And with this, your form has been filled and is ready for submission. It is important that you recheck the form once again to omit any mistakes that you might have made.
Then, click on the ‘Buy’ button to proceed with the payment details. You may now print out the form, but make sure that you have safely kept the original copy with you for record-keeping purposes. Make sure to notarize your document before mailing it.
Mail the Form
Mail the letter by USPS Certified Mail. Sending it as ‘Certified’ is best because it guarantees that the company receives it. In case you are busy and don’t have time to drive to the Post Office, you can mail by Letter Carrier online mailing service. They will mail your document and will send you its “Tracking Number” so that you may know when your document reaches its destination.
However, it is very important that you keep a hard copy of the document you sign and send. Also, remember to always keep a copy of the document, certified mail receipt, payment receipt for your record.
